Search and Care provides information, referral and care management to
approximately 320 people each year with seven full-time and three
part-time paid staff, student interns and volunteers. Services address
each client's specific needs. Assistance is provided with respect for
confidentiality and for the right of an individual to make essential
decisions about his or her life.
Care managers take three main steps:

visit the elderly person and arrange for direct care if necessary

determine short and long-term needs by examining the situation in
detail, identifying problems, and helping the client reach solutions
without sacrificing independence

stay in touch by monitoring or helping coordinate the services
We connect our clients to many different services and resources,
including:

Health and mental health care and health-related services
-
Home care
-
Medicaid and other entitlements and benefits
-
Transportation, Meals On Wheels, social day care, telephone reassurance and other
community-based services

Housing
-
Legal and protective services
Within our own program, staff, interns, social work students and
volunteers provide:

Nursing or occupational therapy visits

Social activities
-
Daily money management
-
Escort, shopping and other individualized services
